70s+: hanging at all is a strong sign of healthy aging.

How much oxygen you use at max effort. In a 2018 JAMA Network Open study, the lowest fitness group had a 404% higher all-cause mortality risk than the elite group.

Measure via a sports lab mask test (most accurate) or a quality wearable estimate.

🏋️ Pillar 2 — Strength:

Hit every major muscle group at least 2x/week (2x beats 1x, clearly). 12–20 weekly sets per muscle group is the optimal hypertrophy range.

Sample 4-day split: Push (chest/triceps/front delts) → Pull (back/biceps/rear delts) → Legs → Full-body second hit.

Why it matters beyond looks: muscle loss begins within days of disuse — even in young people — so you're building a muscle reserve for surgeries, injuries, and old age.

  • Near-max benefit:

    2.5–5 hrs vigorous or 5–10 hrs moderate per week (Circulation, 2022 — 100,000+ adults, 30-year follow-up). More is not better; hormesis needs rest to work.

  • Extreme endurance caution:

    An abstract in the Journal of Clinical Oncology linked sustained ultramarathon-level running to colon tumor risk — early signal, cause unknown, and totally unnecessary for longevity benefits.

  • Your recovery dashboard:

    Day-to-day HRV drops over 10–20%, a rising morning heart rate, poor sleep, elevated CK, or a disrupted cortisol rhythm = your body isn't recovering. As Dr. Sinclair says, you can't consistently improve anything you don't measure.
